Job summary

Hartford HealthCare Medical Group in Connecticut is seeking an experienced registered nurse to join our clinical team. Under provider direction, you will deliver office-based nursing care across multiple settings, following the nursing process to assess, plan, intervene and evaluate patient needs.

The role requires CT RN license and CPR certification, about three years of experience in a medical practice, and familiarity with medical software.

Qualifications

• Current CT RN license and CPR certification.
• Three years of experience in a medical practice preferred.
• Experience with medical software.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and initiates appropriate nursing interventions promptly and professionally.
• Assists patients and family members with concern and confidentiality.
