Job Summary
The Nurse-Family Partnership (NFP) Nurse Home Visitor provides comprehensive nursing services to eligible women and their families, adhering to the NFP model, policies, procedures, and standards of Maternal and Family Health Services, Inc.
Essential Duties and Responsibilities
- Conduct home visits with women and families, following the NFP guidelines and utilizing reflective and motivational interviewing techniques.
- Build therapeutic relationships with up to 30 clients, including primiparous and multiparous mothers.
- Assess physical, emotional, social, and environmental needs; assist in goal setting.
- Provide education, support, and resources using various teaching tools.
- Collaborate with other professionals involved in client care.
- Develop nursing diagnoses, client education plans, and evaluate progress.
- Schedule visits, including telehealth options, and participate in supervisory and team meetings.
- Maintain accurate documentation and participate in quality improvement initiatives.
- Support community outreach and develop referral networks.
- Stay informed on healthcare developments to ensure safe, quality services.
- Contribute to organizational and programmatic goals.
Minimum Qualifications
- Bachelor's Degree in Nursing; candidates with an Associate's Degree are expected to enroll in a BSN program within 2 years and complete it within 5 years.
- Experience in maternal/child health or public health preferred.
- Current PA RN License and CPR Certification.
- Child Abuse, PA Criminal Record, and FBI Clearances.
- Valid driver's license, insurance, and access to a vehicle.
- Excellent communication and customer service skills.
- Ability to walk and climb stairs, carrying up to 25 lbs.
