Job description

Job Title: RN in Home Infusion

The RN in Home Infusion provides community-based infusion nursing services to patients as prescribed by their physician. Responsibilities include assessing, planning, and teaching patients from diverse backgrounds and age groups about infusion therapy, equipment, and care procedures.

  1. Per diem, as needed, with typical hours from 8am to 4:30pm or 8:30am to 5pm. No weekends or holidays.
  2. This position requires travel to patients' homes in Luzerne County, PA.
  3. Minimum of three (3) years of RN experience is required.
  4. Experience in IV, Infusion, and Home Health is preferred.
Job Responsibilities:
  • Coordinate with referral sources and obtain necessary patient information, including medical history, demographics, and physical measurements.
  • Initiate and manage prescribed therapies, including scheduling, supply and medication orders, and interdepartmental communication.
  • Verify and document patient information using electronic medical records and direct patient interviews.
  • Communicate referral details to relevant departments and external agencies.
  • Develop and update patient care plans, providing education on therapy and vascular access devices.
  • Perform patient assessments, including venous system evaluation, and communicate concerns to physicians.
  • Prepare medications using aseptic techniques according to manufacturer and policy standards.
  • Perform peripheral venipunctures, midline, and PICC insertions after training and competency validation.
  • Maintain vascular access devices and perform lab venipunctures as required.
  • Collaborate with pharmacists and physicians to monitor patient progress and response.
  • Ensure all necessary consents, paperwork, and prior authorizations are in place before therapy administration.
  • Maintain equipment and ensure compliance with protocols and standards.
