Jefferson Health is seeking a professional nurse to deliver patient-centered care using the nursing process. You will develop care plans with patients and designated partners, addressing physical, psychological, emotional, and social needs in line with cultural values.

You will work with inter-professional teams, educate patients and families, and participate in safety and quality initiatives to improve outcomes. Active RN license in PA is required, BSN preferred within 3 years.

Competenze

Graduation from an accredited nursing program with a specialized diploma.
Active RN license to practice in Pennsylvania.
Strong knowledge of patient education and discharge planning.

Apply nursing process to address physical, psychological, emotional and social patient needs.
Collaborate with the care team and patient/family to plan individualized care.
Use health IT to document care and monitor outcomes for quality improvement.
Participate in committees and apply evidence-based practice to care delivery.
Provide education to patients and family and support safe transitions of care.
