Pediatric Multispecialty Complex Care Schedule: Monday-Friday, Day Shift

Job Summary

The Registered Nurse Navigator serves as a key coordinator and advocate for pediatric patients with complex medical needs across the continuum of care. Working within the Pediatric Multispecialty Complex Care program, this role coordinates care needs for assigned patients and develops comprehensive, individualized care plans that support optimal health outcomes. The RN Navigator partners closely with patients, families, primary care providers, specialists, and interdisciplinary team members to ensure timely, efficient, and patient-centered care delivery.

The RN Navigator facilitates care coordination across multiple specialties and settings, helps patients and families navigate the healthcare system, promotes continuity of care, and serves as a clinical resource and advocate throughout the patient's healthcare journey.

Key Responsibilities
  • Coordinate and manage the care needs of assigned pediatric patients with complex and chronic health conditions across the patient care continuum.
  • Develop, implement, and monitor individualized care plans based on patient and family needs, goals, preferences, and clinical requirements.
  • Partner with primary care physicians, pediatric specialists, advanced practice providers, and other healthcare professionals to facilitate coordinated care delivery.
  • Collaborate with nurses, social workers, care managers, therapists, pharmacists, and community resources to support comprehensive patient care.
  • Ensure appropriate tests, procedures, consultations, treatments, and follow-up services are completed in a timely manner.
  • Monitor patient progress and adjust care plans as needed to address changing clinical and psychosocial needs.
  • Educate patients and families regarding diagnoses, treatment plans, medications, available resources, and self-management strategies.
  • Serve as a patient and family advocate by identifying barriers to care and facilitating solutions that improve access, outcomes, and patient experience.
  • Promote continuity of care during transitions between inpatient, outpatient, specialty, and community-based settings.
  • Balance clinical care needs with financial and resource considerations to support cost-effective, high-quality care.
  • Maintain accurate and timely documentation in accordance with organizational policies, regulatory requirements, and professional practice standards.
  • Participate in quality improvement initiatives and interdisciplinary team activities that enhance patient outcomes and care coordination.
Qualifications
  • Requires an Associates of Nursing (ASN).
  • Bachelors of Nursing (BSN) preferred.
  • Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
  • Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred.
  • Requires Basic Life Support certification through the AHA.
  • Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
  • Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.
  • Requires knowledge of personal computers using various types of word processing and database software.
  • Requires clinical work experience, medical terminology.
  • Requires understanding of medical record requirements, regulations and policies.
  • Pediatric nursing experience preferred but not required.
  • Care coordination, case management, patient navigation, ambulatory care, or complex care management experience preferred
