Registered Nurse – Medical Case Manager. Contract to Hire Opportunity

As a Medical Case Manager RN, you will receive any children taken into DCFS custody within 24 hours, act as the health work’s lead agent, immediately request all medical records, and be aware of any conditions for each child in your custody. Your duties include ensuring immunization, dental and vision care, maintaining complete documentation, and answering questions foster parents may have.

The position is building‑based at our local office in Peoria and operates Monday through Friday, first shift, with no weekends.

Location: Peoria, IL

Hours: 8:30 am – 4:30 pm

Medical Case Manager Responsibilities include:

• Provide case management services to DCFS children.
• Support the needs of children and families and plan services.
• Monitor family progress toward goals in case files.
