Job summary

Porter-Starke Services, Inc. is seeking a Child Case Manager to provide and coordinate case management for children, adolescents, and seriously and persistently mentally ill adults.

You will locate, coordinate, and monitor care and services, including in-home services, skills training, and crisis intervention, while coordinating entrance or discharge from psychiatric facilities. The role emphasizes developing treatment plans, documenting interventions, advocating for clients, and linking them

Qualifications

• Bachelor's degree in psychology, social work, or related human services field is required.
• Valid Driver’s License and automobile insurance.
• Related experience equal to or more than 6 months.
• Experience in crisis intervention and working with children is preferred.

Responsibilities

• Provides and coordinates case management services and manages caseload.
• Monitors client’s services.
• Provides appropriate therapeutic and crisis interventions.
• Prepares, facilitates and reviews group skills training as per treatment plans.
• Documents interventions per departmental standards.
• Initiates and updates client treatment plans per department standards.
• Assists clients in meeting their treatment goals.
• Connects clients to community resources and advocates for clients.
• Coordinates care with internal and external providers.
• Attends staffing, supervision, in-services and staff meetings.
• Performs other duties as assigned.

Job Duties

• Provides documentation of interventions per departmental standards.

The Child Case Manager Must Demonstrate Knowledge Of

• Therapeutic interventions for seriously and persistently mentally ill children.
• Trauma-Informed Care.
• Screening and Assessment for risk of harm to self and others.
• Signs and symptoms of psychiatric disorders.
• Locating, coordinating and accessing appropriate services and entitlements for client.
• Criteria for enrollment and appropriate regulations to assist clients in maintaining compliance with program guidelines.
• Activities of daily living.
• Formulation of treatment plans, ability to monitor progress, and ability to modify plans to meet client needs.
• Behavior disorders, parenting skills, treatment interventions and crisis intervention.

The Child Case Manager Must Demonstrate Skills In

• Assessing the needs of seriously and persistently mentally ill children.
• Documenting interventions.
• Building and maintaining therapeutic rapport with assigned clients.
• Providing education in activities of daily living for clients.
• Assisting clients in accessing entitlements, such as Medicaid, Social Security Income (SSI) or Social Security Disability Income (SSDI), food stamps, and other resources.
• Assisting clients with medication compliance through self-administering psychotropic medications, or assisted administration.
• Advocating for client's needs.
• Group facilitation.
• Typing and basic computer usage.

Education & Experience

• Bachelor’s degree in psychology, social work or related human services field is required.
