How you’ll make an impact in this role

• Clinical Care Implementation: Execute and oversee comprehensive patient care plans, providing vigilant monitoring and real-time documentation to ensure optimal health outcomes and safety.
• Interdisciplinary Coordination: Serve as the primary liaison across all medical disciplines to orchestrate a unified, holistic approach to patient treatment and recovery.
• Precision Order Management: Accurately process and implement physician and nursing orders, maintaining rigorous adherence to clinical protocols and institutional standards.
• Strategic Discharge Advocacy: Assess and coordinate complex discharge planning in partnership with the healthcare team to facilitate seamless and safe transitions of care.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse obtained prior to hire date or job transfer date required.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
