Job summary

Tallgrass Healthcare Campus in Junction City, KS is seeking an experienced MDS Coordinator (RN) to lead resident assessments, develop individualized care plans, and coordinate interprofessional care. This role emphasizes accuracy, compliance, and resident-centered care within a supportive team.

The successful candidate will hold Kansas RN licensure, bring 2+ years in gerontological nursing, and excel in communication, organization, and teamwork.

Qualifications

• Current licensure as a Registered Nurse in the State of Kansas or Compact Licensure.
• Minimum 2 years of experience in gerontological nursing.
• Strong clinical skills and knowledge of gerontological nursing practices.
• Excellent communication, organizational, and interpersonal skills.

Responsibilities

• Conduct initial and comprehensive resident assessments using the MDS and develop individualized care plans.
• Lead and participate in care plan conferences to ensure comprehensive care.
• Collaborate with the interdisciplinary team to develop and implement resident care plans.
• Identify and prioritize resident needs, and coordinate care to meet those needs.
• Maintain accurate and timely records of resident assessments, care plans, and progress.
• Participate in quality improvement initiatives to enhance resident outcomes and care.

Requirements:
