Job Summary

We are seeking a dedicated and detail-oriented Registered Nurse/MDS Coordinator to join our healthcare team in Henderson, KY. The ideal candidate will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments to ensure accurate and timely documentation of resident care and compliance with regulatory requirements.

Key Responsibilities
  • Conduct comprehensive MDS assessments for residents in accordance with federal and state guidelines.
  • Coordinate with interdisciplinary team members to gather necessary clinical information.
  • Ensure accuracy and completeness of MDS documentation to support quality care and reimbursement.
  • Monitor resident care plans and update as necessary based on assessment findings.
  • Assist in the development and implementation of care plans tailored to individual resident needs.
  • Maintain compliance with all regulatory requirements related to MDS and resident care documentation.
  • Provide education and training to staff regarding MDS processes and documentation standards.
  • Collaborate with nursing staff, physicians, and other healthcare professionals to optimize resident outcomes.
  • Participate in quality improvement initiatives and audits related to MDS and resident care.
Qualifications
  • Current and valid Registered Nurse (RN) license in the state of Kentucky.
