Qualifications and Responsibilities:

• Associates or Bachelor's degree in Nursing
• Active, unencumbered New Jersey RN license
• At least 6 months experience as an MDS nurse in a long-term care setting (preferred)
• Conduct and coordinate MDS assessments per federal and state regulations
• Collaborate with interdisciplinary teams for comprehensive resident assessments
• Ensure accuracy and completeness of assessments for care planning and compliance
• Stay updated on MDS regulation changes and implement adjustments
• Participate in care planning meetings and develop individualized care plans
• Monitor resident progress and update assessments accordingly
• Work with staff to facilitate billing and reimbursement processes
• Provide staff education on MDS assessments and documentation
