Job summary

Pathways Healthcare is seeking a Registered Nurse or Licensed Practical Nurse for Home Health services in Massachusetts. The role covers Mansfield and surrounding areas with a flexible schedule and per diem arrangement.

Office based in Westwood, MA with travel to patients’ homes. The ideal candidate holds an active nursing license in MA, 2+ years of geriatric home care experience, a nursing degree, and strong communication with a patient‑centered approach.

Qualifications

• Must hold an active Nursing license in the state where the position is based.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create robust plans of care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them.
