ComForCare Home Health Care - Northern CO in Greeley, Colorado is seeking an experienced Registered Nurse in Home Care to manage client caseloads, guide a team of caregivers and provide high-quality home-based nursing.

The role offers a pay range of $47 to $52 per hour based on experience, a Monday–Friday 8 a.m.–5 p.m. schedule with some holidays and weekend rotation, and a full-time position with competitive benefits including health insurance and 401(k) options.

Formación

• Active, unencumbered RN license in Colorado.
• At least 1 year of home care or home health nursing experience.
• Case management, care coordination or caregiver supervision experience (preferred).
• Current BLS/CPR certification.
• Valid driver’s license and reliable transportation.
• Oasis Documentation.
• Strong clinical judgment, organization and communication skills, plus a genuine passion for client-centered care.

Responsabilidades

• Client intake and assessments: Complete initial and ongoing in-home nursing assessments of each client’s physical, cognitive, psychosocial and home safety needs.
• Care plan development: Create and update individualized plans of care and ensure caregivers understand and follow them.
• Caseload management: Oversee a caseload of home care clients, track changes and adjust services as needed.
• Caregiver supervision and training: Conduct supervisory visits and complete competency checks.
• Care coordination: Serve as the main clinical contact for families, physicians, hospitals and community partners; make referrals when needed.
• Documentation and compliance: Keep accurate, timely client records and follow Colorado home care regulations, infection control and HIPAA guidelines.
• On-call support: Help resolve client and caregiver concerns quickly and professionally.
