Overview

Are you an RN looking to deliver high-acuity care outside the walls of a hospital? Our Hospital at Home program offers a unique opportunity to provide hospital-level nursing care directly in patients' homes—combining clinical excellence with autonomy, flexibility, and meaningful patient relationships.

Responsibilities

• Start and manage the patient's plan of care, including:
  • Nursing assessments, medication reconciliation, and safety evaluations
  • Implementation of standardized clinical protocols
  • Administer prescribed medications (primarily IV), draw blood, insert foley catheters, and deliver therapeutic interventions
  • Educate patients and caregivers on their condition, prescribed medications, nursing interventions, and care plans
  • Monitor, identify, and act on changes in patient condition
  • Evaluate and revise assessments and care plans based on patient needs and home environment
  • Document all assessments, interventions, and care coordination activities in accordance with regulatory guidelines
  • Participate in on-call rotation
  • Perform all other duties as assigned

Requirements
