Signature HealthCARE is seeking an experienced Registered Nurse in Clarksville, TN to join our skilled nursing facility. Provide direct nursing care, supervise aides, and coordinate admissions and discharges. Must have active RN license and strong clinical skills. Supervisory experience and familiarity with EHRs are a plus.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration and documentation.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently and exercise sound clinical judgment.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure dignity, comfort and safety.
• Supervise nursing assistants and medication aides per policy.
• Monitor residents' status and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers and discharges.
• Participate in quality assurance, staff education and surveys.
• Perform other duties as assigned by the Director of Nursing.
