Job summary

Signature HealthCARE of Clarksville is seeking a seasoned RN to provide direct nursing care, supervise nursing staff, and coordinate resident care within a 120-bed facility. You’ll monitor health status, document care, and collaborate with the interdisciplinary team to ensure quality outcomes.

The role requires an active RN license in the state, preferred CPR/BLS, and 1+ year of clinical nursing experience, especially in long-term care. EHR and MS Office familiarity are beneficial.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (AHA or ARC).
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Experience with EHR systems.
• Familiarity with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents’ status and notify physician or family of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in QA activities and staff education.
