Job summary

Northwestern Medicine is seeking a registered nurse in Illinois to assess patient status on admission, organize health data, and collaborate with a multidisciplinary team to develop and update individualized plans of care. Candidates should have a BSN, IL RN license, and BLS certification, with ADN acceptable if a BSN will be pursued within three years.

Qualifications

• BSN required; Illinois RN license (Registered Professional Nurse)
• American Heart Association Basic Life Support (BLS) certification
• ADN acceptable with commitment to obtain BSN within three years of employment
• ER: ACLS, PALS/ENPC & TNCC within one year of hire
• Pediatric ER: TNCC within one year of hire
• Non-Invasive Cardiology: ACLS certification
• Strong interpersonal skills and ability to collaborate with others
• Experience in the specified clinical areas is preferred

Responsibilities

• Assess and record patient status upon admission or entry.
• Organize assessment data to be accurate, complete and accessible.
• Identify problems needing immediate attention and validate diagnoses.
• Document diagnoses to support expected outcomes and care plan.
• Initiate and update the Plan of Care with age-appropriate considerations.
• Discuss and obtain agreement on the plan with patient/family.
• Begin discharge planning at admission and coordinate with care team.
• Provide patient and family education based on evidence-based practice.
• Communicate and coordinate with patient/family and care team regarding progress.
• Contribute to multidisciplinary meetings for feedback and planning.
• Refer patients as needed for continuity of care.
• Review health data to keep Plan of Care current and effective
