Job Description

In hospital setting, accountable for utilizing the nursing process. Responsible for performing patient care, delegating patient care tasks and supervising other patient care staff. Responsible for coordinating the plan of care for a group of assigned patients and consulting with other members of the healthcare team to ensure optimal patient outcomes. Demonstrates proficiency and adheres to policies, rules, guidelines, and procedures.

Virtual Setting

Functions independently, utilizing the nursing process. Interprets data with increasing judgment, prioritizes appropriately, and initiates actions to improve nursing practice and patient care. Serves as a resource to colleagues, delegates tasks, and assumes preceptor responsibilities as needed. Provides tele-nursing care across departments, monitoring patients with intelligent software, alerting to changes, and partnering with bedside staff to address concerns. Located remotely at the Virtual Care Center in Lees Summit, focusing on early detection and complication reduction.

In Clinic Setting

• Provide professional nursing care, including telephone triage, patient flow, and care coordination.
• Schedule appointments, identify learning needs, provide advice, administer medications, and perform duties based on clinic needs.
• Delegate tasks appropriately, serve as a subject matter expert, and manage patient education and documentation.

In Community Hospital Setting

• Manage care within established nursing standards, triage patients, and initiate care to stabilize patients.
• Collect patient history, perform lab testing, and report adverse treatments following policies.
• Participate in QAPI activities and maintain accreditation standards.

In Population Health Setting

• Advocate for patients, develop care plans, coordinate care, and communicate across disciplines.
• Perform assessments, collaborate on care plans, and monitor patient progress during transitions.
• Educate patients and families, review medical information, and ensure quality of care and HIPAA compliance.

In Hospital In Your Home Setting

• Provide in-home nursing, assess patient needs, and collaborate with the healthcare team.
• Document visits, follow orders, and abide by policies and procedures.

In ASC Setting

• Provide nursing care, triage, schedule appointments, and perform clinical duties as needed.
• Delegate tasks, serve as a subject matter expert, and participate in quality activities.

Job Requirements

Applicable Experience: 1 year
