Schedule: Mon.-Fri. 8:00am - 4:30pm

This position is eligible for a $5,000 retention bonus.

Job Summary:

The Providence Center ICT Team 1F is seeking a motivated, team-oriented Registered Nurse to assess and facilitate care for adults with co-occurring conditions, medical complexities, and unstable living situations. Experience with chronically homeless and addiction populations, especially within our new "CCBHC" (Certified Community Behavioral Health Clinic) capacity, is a plus.

Responsibilities:
  • Triage and assess assigned consumers.
  • Evaluate health data through interviews, observation, records, and team input.
  • Develop individualized care plans.
  • Perform integrated care following medical protocols.
  • Identify and aim to improve health outcomes.
  • Implement and evaluate care plans, updating as needed.
  • Support consumers with daily living issues, including community and home visits.
  • Educate consumers and families about disease, treatment, and medication effects.
  • Administer medications as ordered, including injections and oral medications.
  • Monitor lab results and document accordingly.
  • Coordinate with treatment team and pharmacies on medication management.
  • Manage hospitalizations, follow-up, and discharge processes.
  • Maintain accurate records and complete assessments.
  • Collaborate with staff and external agencies.
  • Participate in training, committees, and onboarding new staff.
  • May share on-call duties with team members.
Requirements:
  • Associate’s degree in Nursing required; BSN preferred.
  • Rhode Island RN license required.
  • Experience in Human Services or outpatient settings preferred.
  • Experience with psychiatric and substance use disorders a plus.
  • Valid driver’s license, registration, and insurance required, as position may require driving.
