Registered Nurse ICT Team 1F - $5,000 Retention Bonus

This position is eligible for a $5,000 retention bonus.

Job Summary:

The Providence Center is seeking a motivated, team-oriented Registered Nurse for the ICT Team 1F. The role involves assessing and supporting adults with co-occurring conditions, medical complexities, and unstable living arrangements. Experience with chronically homeless individuals and addictions populations, especially within our new "CCBHC" (Certified Community Behavioral Health Clinic) capacity, is a plus.

Responsibilities:
  1. Triage and assess consumers.
  2. Evaluate health care data through interviews, observation, record review, and team input.
  3. Develop individualized care plans.
  4. Perform integrated care following medical protocols.
  5. Identify expected outcomes and aim to improve overall health.
  6. Implement and evaluate care plans, updating as needed.
  7. Support consumers with daily living, including community and home visits.
  8. Educate consumers and families about diseases, treatments, and medications.
  9. Administer medications as prescribed, including injections and oral medications.
  10. Monitor lab results and document accordingly.
  11. Coordinate with treatment teams and pharmacies regarding medication management.
  12. Manage hospitalization activities and follow-ups post-discharge.
  13. Maintain accurate records and complete assessments.
  14. Collaborate with staff and external agencies.
  15. Participate in training, conferences, and serve on committees.
  16. Assist in training new staff and students. May share on-call duties.
Requirements:
  • Associate’s degree in Nursing required; Bachelor’s (BSN) preferred.
  • Rhode Island RN license required.
  • Experience in Human Services or outpatient settings preferred.
  • Experience with psychiatric and substance use disorder patients is a plus.
  • Valid driver’s license, registration, insurance, and reliable vehicle may be required.
