Job description

This position is eligible for a $5,000 retention bonus.

Job Summary:

The Providence Center ICT Team 1F is seeking a motivated, team-oriented Registered Nurse to assess and facilitate care for adults with co-occurring conditions and medical complexities, including those experiencing unstable living conditions. Experience with chronically homeless populations and addictions, especially within our new "CCBHC" (Certified Community Behavioral Health Clinic) capacity, is a plus.

Duties & Responsibilities:
  1. Triage and assess consumers assigned to the team.
  2. Evaluate client health data through interviews, observation, record review, and team input.
  3. Develop individualized care plans.
  4. Perform integrated care following medical protocols.
  5. Identify expected outcomes to improve overall health.
  6. Implement and evaluate care plans, updating as necessary.
  7. Support consumers with daily living issues through community and home visits.
  8. Educate consumers and families about disease processes, treatments, and medications.
  9. Administer medications as per doctor’s orders, including injections and oral medications, following the 6 rights of medication administration.
  10. Monitor lab results and document findings.
  11. Ensure accurate documentation of externally prescribed medications in EMR.
  12. Liaise with treatment teams and pharmacies regarding medication changes.
  13. Coordinate hospitalization activities and follow-up care post-discharge.
  14. Complete medication reconciliations and review discharge paperwork.
  15. Collaborate with staff and perform recordkeeping per agency standards.
  16. Complete assessments and maintain relationships with consumers, staff, and external agencies.
  17. Attend trainings, case presentations, and conferences.
  18. Participate in departmental activities, serve on committees, and assist in training new staff and students.
  19. May share on-call responsibilities with other staff.
Requirements:
  • Associate’s degree in Nursing required; Bachelor's degree (BSN) preferred.
  • Rhode Island RN license required.
  • Experience in Human Services or outpatient settings preferred.
  • Bilingual Spanish skills are a plus.
  • Experience with psychiatric and substance use disorder patients is advantageous.
  • Valid driver’s license, registration, insurance, and a reliable vehicle are required, as driving may be necessary.
