Job summary

Amedisys is seeking a registered nurse for PRN home health roles in Alabama. You will assess, plan, implement and coordinate care for patients in their homes, collaborating with the care team to ensure high-quality outcomes.

Responsibilities include patient assessments, care plan development, and education for families. CPR-certified RNs with a valid license and reliable transportation are encouraged to apply.

Qualifications

• Current RN license, state-specific as applicable.
• One year of nursing experience preferred; new graduates considered for RN program.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Perform initial nursing assessment on new admissions.
• Develop and maintain the plan of care and ensure continuity of patient care.
• Regularly communicate patient progress to the clinical manager, physician, and care team.
• Maintain accurate patient care progress records and timely documentation.
• Record pain and symptom management changes and outcomes as appropriate.
• Educate families on patient condition and care needs.
• Make referrals to other hospice care disciplines as needed or document rationale.
• Perform on-call responsibilities for patients and families as assigned.
• Perform other duties as assigned.

Responsibilities

• Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of nursing experience preferred. If less than one year of nursing experience, you can be considered for the RN new graduate program.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
