VitalCaring is seeking a Hospice Registered Nurse (RN) based in Hammond, LA, serving the North Shore. You will lead field-based, patient- and family-centered end-of-life care, collaborating with a multidisciplinary team to ensure comfort, dignity, and clinical excellence in home and facility settings.

Responsibilities include comprehensive assessments, care planning with symptom management, acting as care coordinator, educating families, and maintaining compliant documentation with IDT

Qualifications

• Active RN license (or compact eligibility) required.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care.
• Ability to manage a caseload independently.
• Excellent communication in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.
• Experience with Homecare Homebase (HCHB) preferred.

Role Overview

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

What You’ll Do

Lead Patient-Centered Hospice Care

• Conduct comprehensive patient and family assessments
• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with a focus on pain and symptom management

Serve as the Care Coordinator

• Act as case manager, coordinating care across the interdisciplinary team
• Collaborate with physicians, social workers, chaplains, and aides
• Monitor patient condition and adjust care plans to maintain comfort

Support Patients & Families

• Provide education, emotional support, and guidance throughout the journey
• Help families navigate complex decisions with empathy and clarity
• Build meaningful, trusting relationships in deeply personal moments

Drive Quality & Compliance

• Maintain accurate, timely documentation
• Participate in IDT meetings and care planning
• Support quality and compliance standards

What You Bring

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
