VitalCaring is seeking a Hospice Registered Nurse (RN) to lead the plan of care, manage symptoms, and deliver comfort-focused, goal-aligned care for patients at the end of life in the Hammond, LA area.

Work involves field-based, one-on-one visits to patients and families, coordinating with an interdisciplinary team and providing education, emotional support, and compassionate guidance throughout the journey.

Qualifications

• Active RN license (or compact eligibility) in good standing.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver's license and reliable transportation.

Responsibilities

• Lead comprehensive patient and family assessments.
• Develop and manage individualized, comfort-focused care plans.

Role Overview

What You'll Do

Lead Patient-Centered Hospice Care

• Conduct comprehensive patient and family assessments
• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with a focus on pain and symptom management

Serve as the Care Coordinator

• Act as case manager, coordinating care across the interdisciplinary team
• Collaborate with physicians, social workers, chaplains, and aides
• Monitor patient condition and adjust care plans to maintain comfort

Support Patients & Families

• Provide education, emotional support, and guidance throughout the journey
• Help families navigate complex decisions with empathy and clarity
• Build meaningful, trusting relationships in deeply personal moments

Drive Quality & Compliance

• Maintain accurate, timely documentation
• Participate in IDT meetings and care planning
• Support quality and compliance standards

What You Bring

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
