Job summary

VitalCaring Group seeks a Hospice Registered Nurse (RN) to lead field-based, patient- and family-centered end-of-life care. You’ll manage symptoms, plans of care, and comfort-focused treatment in patients’ homes and communities with autonomy and interdisciplinary support.

As the clinical leader and advocate for your patients, you’ll build trust, coordinate with physicians, social workers, chaplains, and aides, and ensure compassionate, high-quality care aligned with each patient’s goals.

Qualifications

  • Active RN license (or compact eligibility).
  • Graduate of an accredited nursing program.
  • Minimum 1 year of clinical nursing experience.
  • Strong clinical judgment and compassionate care approach.
  • Ability to manage a caseload independently.
  • Excellent communication skills in sensitive situations.
  • Basic EMR proficiency.
  • Valid driver’s license and reliable transportation.

Responsibilities

  • Lead patient-centered hospice care and assess patients and families.
  • Develop and manage individualized, comfort-focused care plans.
  • Deliver skilled nursing care focusing on pain and symptom management.
  • Act as case manager coordinating care with the interdisciplinary team.
  • Monitor patient condition and adjust care plans to maintain comfort.
  • Provide education, emotional support to families.
  • Navigate complex decisions with empathy and clarity.
  • Document accurately and timely; participate in IDT meetings.
  • Support quality and compliance standards.

Role Overview

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

What You’ll Do
Lead Patient-Centered Hospice Care
  • Conduct comprehensive patient and family assessments
  • Develop and manage individualized, comfort-focused care plans
  • Deliver skilled nursing care with a focus on pain and symptom management
Serve as the Care Coordinator
  • Act as case manager, coordinating care across the interdisciplinary team
  • Collaborate with physicians, social workers, chaplains, and aides
  • Monitor patient condition and adjust care plans to maintain comfort
Support Patients & Families
  • Provide education, emotional support, and guidance throughout the journey
  • Help families navigate complex decisions with empathy and clarity
  • Build meaningful, trusting relationships in deeply personal moments
Drive Quality & Compliance
  • Maintain accurate, timely documentation
  • Participate in IDT meetings and care planning
  • Support quality and compliance standards
What You Bring
  • Active RN license (or compact eligibility)
