Job summary

Bluegrass Care Navigators is seeking a dedicated Hospice Flex RN to join our team providing compassionate care to patients at home or in facilities across Kentucky. You will support Hospice RN Case Managers, manage visits, and participate in on-call rotations to ensure continuous high-quality care.

Qualifications

• Graduate of an accredited school of registered nursing.
• Licensed as a Registered Nurse in Kentucky or via a Compact State.
• Minimum of 1 year general nursing experience preferred; new grads considered.
• Demonstrates professional, compassionate patient care and cultural sensitivity.
• CPR certification required.

Responsibilities

• Provide care in home or facility settings as part of Hospice Flex RN rotation.
• Support Hospice RN Case Managers and manage patient visits.
• Assist with admissions, referrals, and on-call rotation.
• Demonstrate strong assessment skills and disease process understanding.
• Communicate effectively with patients, families, and interdisciplinary team.

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Position Qualifications

• Graduateof an accredited school of registered nursing.
• Currently licensed as a Registered NursethroughtheKentucky Board of Nursingor through a participating CompactStateBoard of Nursing.
• Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered.
• Demonstrates strong clinical nursing skills and the ability tomaintainprofessionalism during emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
• Adheres to accepted professional, ethical, and legal standards of nursing practice.
• Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
• Maintains current CPR certification.
• Must be a licensed driver with an automobile in good working order, that isinsuredin accordance withstateminimumrequirements.
