Job summary

Care New England Health System in Rhode Island is seeking a compassionate Hospice RN to manage a patient caseload as part of an interdisciplinary team. You will work to promote optimal outcomes through assessment, planning, and coordination of care in a home-based setting.

Responsibilities include comprehensive assessments, care planning, coordinating referrals, and educating patients and families. Strong communication and clinical judgment are essential.

Qualifications

• Requires RI RN license.
• BSN strongly preferred.
• At least 1 year of clinical nursing experience (hospice preferred).
• Current CPR certification.
• Valid driver's license and automobile liability insurance.
