Job summary

Lakes Region Visiting Nurse Association in Meredith, NH, is seeking a Hospice Registered Nurse to act as the Case Manager for hospice clients. The RN coordinates with the Interdisciplinary Team to create individualized plans of care for patients and families in the community setting.

Experience in hospice, palliative, or end-of-life care is required. Full‑time, part‑time, or per‑diem hours are available with excellent salary and benefits.

Qualifications

  • Graduate of an accredited school of nursing.
  • 1-2 years of recent patient-centered care/nursing experience.
  • Current state licensure, CPR certification, and a valid driver’s license.
  • Bachelor’s degree in nursing with one year of hospice care experience preferred.

Responsibilities

  • Completes an initial assessment of patient and family and shares findings with the IDG.
  • Completes HIS within two days of hospice election and again at discharge.
  • Completes nursing assessment on admission and election of hospice.
  • Coordinates with the IDG to complete all disciplines’ assessments within five days of election.
  • Develops a care plan with IDG that addresses client goals and needs.

Job Title: Registered Nurse - Hospice

Location: Meredith, NH, and surrounding towns

Job Type: Full-Time

Position Summary

The Hospice Registered Nurse acts as the Case Manager for their hospice clients, working closely with all members of the Interdisciplinary Team (IDG) to create a highly personal and individualized plan of care. The RN organizes and directs services for hospice clients, their families, and caregivers based on identified needs and goals. Experience in community health and hospice, palliative or end‑of‑life care is required. Full‑time, part‑time, or per‑diem hours are available with excellent salary and benefits.

Primary Duties and Responsibilities
  • Completes an initial assessment of patient and family to determine immediate needs and brings them to the IDG.
  • Completes the HIS within two days of the election of hospice and again at discharge, in accordance with the hospice CoP’s.
  • Completes nursing comprehensive assessment on admission and election of hospice.
  • Coordinates with the IDG to ensure that all disciplines complete their assessments within five days of the election of hospice.
  • Develops a care plan with the support of the IDG that establishes goals based on the client’s diagnosis, physical, emotional, and spiritual needs.
Communication
  • Prepares clinical notes and communicates with the primary physician/hospice medical director every fourteen days.
  • Communicates with the physician regarding the patient’s needs and reports changes in condition.
  • Obtains/receives physicians orders as required.
  • Communicates with the entire interdisciplinary team to coordinate and update the client’s plan of care.
  • Ensures that all required documentation is done and submitted on time in compliance with the hospice CoP’s.
Qualifications, Skills, And Competencies
  • Graduate of an accredited school of nursing.
  • 1‑2 years of recent patient‑centered care/nursing experience.
  • Current state licensure, CPR certification, and a valid driver’s license.
  • Bachelor’s degree in nursing with one year of hospice care experience preferred.
  • Strong observation, communication, and problem‑solving skills.
  • Physical capability to lift patients and equipment, with considerable walking, standing, reaching, bending, and kneeling.

Supervisory Responsibilities
  • Supervise and coordinate care with LPNs and LNAs.
