Job summary

Haven Hospice in Lake City, FL is seeking a passionate Hospice Registered Nurse to deliver patient-centered care in a home setting. This PRN role covers a 5-county area with a Monday–Friday schedule and requires at least 2 shifts every biweekly pay period.

You will assess needs, develop care plans, administer treatments, manage symptoms, educate families, and collaborate with an interdisciplinary team. A nursing license, CPR, and hospice experience are preferred; driver’s license and reliable

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
