Description

POSTITION SUMMARY:

The HomeCare Registered Nurse will manage primarily HomeCare patients but will be cross trained to provide Hospice care as well. The Registered Nurse will provide skilled nursing care to clients in their home. This role is performed in accordance with the physician orders and the plan of care. The Registered Nurse is responsible for utilizing the nursing process in the development of treatment plans, with established goals, implementation, monitoring and evaluation of an assigned patient case load to provide quality care appropriate to clinical needs. The Registered Nurse will work as part of an interdisciplinary team, providing a team approach to high quality patient care. Schedule will include rotating weekends and ON CALL nursing coverage

DUTIES & RESPONSIBILITIES:

Provides professional nursing expertise to patient/families in the home environment by:

Performing comprehensive initial and ongoing assessments of patients/families.

Using assessment skills to determine patient’s physical and mental status of environment and social support.

Maintains patient’s immediate environment to be safe and clean.

Identifying and assessing patients’/families’ health and social problems, potential problem areas, needs and resources of individuals, families and significant others.

Planning, implementing, and evaluating nursing care for patients and families.

Serves as the professional health care provider and coordinator of the Schoolcraft Memorial Homecare team in planning, developing and evaluating patient care by:

Developing and modifying a plan of care for the patient and family based on a thorough assessment of problems, potential problems and the medical plan of treatment.

Planning nursing interventions that are consistent with the standards of nursing practice.

Developing measurable patient-centered goals in collaboration with the patient and family.

Coordinating home care and interdepartmental services, medical care, community resources, and resources within the home in developing the patient/family care plan.

Implementing services in accordance with the established care plan and in compliance with the third-party regulators, certified home care requirements, Schoolcraft Memorial HomeCare policies, procedures, protocols, and standards of nursing practice, and all Federal, State and Local laws/regulations.

Assisting in coordinating and participating in multi-disciplinary clinical conferences to assure that the team efforts effectively complement one another and support the objectives outlined in the care plan.

Coordinating patient care and community resources.

Preparing, administering, and documenting medications and treatments as prescribed in the Physician’s Plan of Treatment and Schoolcraft Memorial Home Care policies. Maintaining continuous open communication with the physician for optimum coordination of care.

Accompanies patient to physician appointments prn.

Teaching and demonstrating care to the patient/family, utilizing established, available patient education, principles, guidelines, and resources; assessing effectiveness or patient/family teaching toward fostering independence in self care.

Providing supervision to ancillary health care workers, including but not limited to: Schoolcraft Memorial HomeCare LPN’s, Aides and students, assuring appropriate implementation and follow through of the Plan of Treatment.

Developing the Aide Assignment plan, and supervising the Aide assuring appropriate implementation and follow through of the Aide Assignments.

Delegating appropriate duties to the LPN for appropriate patients using nursing judgment and supervising the LPN assuring appropriate implementation and follow through of the Plan of Treatment.

Acting as primary nurse for a patient as directed by Nursing Supervisor.

Evaluating the patient/family response to the Plan of Treatment.

Using patient and family centered goals as criteria for evaluating and modifying the Plan of Treatment.

Provides comprehensive documentation in the patient’s medical record that is reflective of the patient/family clinical progress.

Completing medical records according to professional standards, third party regulatory requirements and Schoolcraft Memorial HomeCare policy.

Maintaining current signed and accurate physician orders on all active patients.

Participating in obtaining information and signatures to complete fee agreements and insurance forms.

Completing other Schoolcraft Memorial HomeCare records and reports in an accurate and timely manner.

Demonstrates the ability to organize and manage patient care activities.

Willing and able to meet consistently the patient visit frequency expectations in accordance with the Schoolcraft Memorial HomeCare’s productivity guidelines.

Organizing work to adequately perform the function of caseload management.

Contributes to professional standards of care at Schoolcraft Memorial HomeCare by:

Collaborating with Schoolcraft Memorial HomeCare management to identify issues that require further exploration.

Participating in and supporting the Schoolcraft Memorial HomeCare Quality Improvement Program.

Participating in Schoolcraft Memorial HomeCare meetings, activities, programs, inservices and continuing education classes.

Remaining flexible to changes in Schoolcraft Memorial HomeCare policy and procedure and in daily scheduling.

Maintaining collegial relationships with other disciplines, interpreting the role and responsibilities of the Schoolcraft Memorial HomeCare RN to others.

Maintaining regular work attendance.

Enforces Schoolcraft Memorial HomeCare policies and procedures by:

Communicating and interpreting the policies and procedures to members of the Schoolcraft Memorial HomeCare team, patients and families.

Communicating with Schoolcraft Memorial HomeCare management if policies and/or procedures are violated.

Follows agency policy and procedures for emergencies.

Contributes to effective client relations by assisting patient, families, co-workers and other health care members to resolve expressed concerns and by demonstrating a positive and helpful attitude.

Informs the patient upon admission to Schoolcraft Memorial HomeCare of the mechanism for receiving, reviewing and resolving patient complaints.

Minimizes infectious diseases among employees and patients by following infectioncontrol policies and protocols. Attends on-going inservices education programs regarding safety issues in the home, equipment, and universal precautions.

Keeps information confidential by adhering to the terms of Schoolcraft Memorial Human Resources policy concerning confidential information.

Demonstrate appropriateness in meeting objectives in age-specifics.

Contributes to organizational operations by performing other related duties as maybe required.

Qualifications

Currently licensed as a Registered Nurse in the State of Michigan

One year of recent experience, home health care preferred

Current CPR certification

Able to provide own transportation to patients’ homes

Good mental and physical health

Satisfactory references

Able to work beyond scheduled hours as needed.
