Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team. You will provide skilled nursing and rehabilitative care to clients in their homes, collaborating with a multidisciplinary team to ensure compassionate, high-quality care that enables patients to remain at home.

Successful candidates will demonstrate strong assessment skills, experience in acute/post-acute care, and the ability to make real-time clinical decisions.

Qualifications

• Active RN license in good standing in MA.
• Graduate from an accredited nursing program.
• Minimum 1 year of hands-on medical experience with adult/geriatric populations in clinical settings.

Responsibilities

• Perform comprehensive head-to-toe assessments and identify changes in patient condition.
• Triage and independently manage urgent home-based clinical situations.
• Conduct medication reconciliation, patient teaching, and safety monitoring.
• Perform wound assessment, care, and documentation in home health visits.

You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. • Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
• Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation

Qualifications for a Registered BAYADA Nurse:

• A current nursing license in good standing in the state of Massachusetts
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• At least one year of current or recent hands‑on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub‑acute care or rehab units (medical or skilled nursing settings).
