Job summary

Jefferson Health at Home by BAYADA is seeking a Full Time Registered Nurse (RN) to join our home health visit team. You will deliver skilled nursing and rehabilitative care in patients' homes, collaborating with a multidisciplinary team to support staying at home with compassionate, high-quality care.

You will perform head-to-toe assessments, manage acute and post-acute conditions, and provide medication reconciliation and safety monitoring in diverse home environments.

Qualifications

• Current nursing license in good standing in Pennsylvania
• Graduation from an accredited nursing program
• At least 1 year of hands-on medical experience with adult/geriatric populations in acute care, ER, home health, hospice, ICU, med-surg or rehab settings

Responsibilities

• Perform comprehensive head-to-toe assessments and identify subtle changes in patient condition
• Provide skilled nursing and rehabilitative care in the home setting
• Triage and manage urgent clinical situations independently
• Conduct medication reconciliation, teaching, and safety monitoring

Jefferson Health at Home by BAYADA is currently seeking a Full Time Registered Nurse (RN) to join our home health visit team. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. • Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, teaching, and safety monitoring

Qualifications for a Registered Jefferson Health at Home by BAYADA Nurse

• A current nursing license in good standing in the state of Pennsylvania
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• At least one year of current or recent hands‑on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub‑acute care or rehab units (medical or skilled nursing settings).

Pay: Weekly

Base Pay, Depending On Qualifications And Subject To Negotiations

$60-$65 per point ($93,600-$101,400 annual earning potential)
