Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to provide in-home health visits across Monroe County, PA. This PRN/Part-Time/Full-Time opportunity offers 1:1 patient care and flexible scheduling to support work-life balance.

Responsibilities include following care plans, administering medications, wound care, and coordinating with clinical teams. PA RN license and at least 1 year of clinical experience are required.

Qualifications

• Active Pennsylvania RN license.
• Minimum one year of recent, verifiable clinical (medical/surgical) experience.
• Home care experience preferred but not required.
• Graduate from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Proficient computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN)

• A current license as a Registered Nurse in Pennsylvania.

• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information

Base Pay, depending upon qualifications and subject to negotiation:

$52.00 - $64.00 per point
