BAYADA Home Health Care in Pennsylvania is hiring a Registered Nurse (RN) for home health visits across Schuylkill County. Flexible schedules, 1:1 care, and a strong work/life balance are offered.

As an RN, you will follow care plans, visit clients, administer medications, monitor conditions, document care, and coordinate with the care team. A PA RN license and at least 1 year of clinical experience are required; home care experience preferred.

Competenze

• Active Pennsylvania RN license is required.
• Minimum 1 year of recent clinical (medical/surgical) experience.
• Home health experience preferred but not required.
• Graduate from an accredited nursing program with transcript or diploma.

Mansioni

• Follow a designated care plan according to patient needs.
• Make home visits to clients in the geographic territory.
• Administer medications, wound care, treatments and procedures.
• Monitor clients' conditions and report changes to the manager.
• Follow up with, execute, and document doctors' orders.

Additional Information:
Base Pay, depending upon qualifications and subject to negotiation:
$52.00 - $64.00 per point ($67,600-$83,200 annually estimated)

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessaryCase management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in Pennsylvania.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

$52.00 - $64.00 per point
