Job summary

BAYADA Home Health Care is seeking a compassionate RN to provide one-on-one home health visits across Northampton County, PA. This PRN/Part-Time/Full-Time opportunity offers flexible schedules, 1:1 care, and a commitment to work-life balance.

Applicants must hold a PA RN license and have at least 1 year of clinical experience, with home health or visiting nurse experience preferred. Travel to patient homes is required, and BAYADA offers comprehensive benefits and career growth opportunities.

Qualifications

• Active PA RN license with current registration.

• Ability to travel to client homes as needed.

Responsibilities

• Follow a designated care plan based on patient needs.
• Make home visits to clients across designated territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to Clinical/Client Services Manager.
• Follow up on doctors' orders and complete proper documentation.
• Perform client assessments as necessary and coordinate case management.
• Document observations and interventions in EMR with accuracy.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in Pennsylvania.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information:
Base Pay, depending upon qualifications and subject to negotiation:
$52.00 - $64.00 per point
