Job summary

BAYADA Home Health Care seeks a Registered Nurse (RN) to perform home health visits in Bushkill and surrounding areas, delivering one-to-one care with flexible schedules and a strong commitment to patient well-being.

The RN will follow care plans, administer medications, monitor patient status, and document care using EMR tools. PA licensure and recent clinical experience are required; home care experience is preferred.

Qualifications

• Current PA RN license is required.
• One year of recent clinical experience preferred.
• Home care experience is a plus but not required.

Responsibilities

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessaryCase management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in Pennsylvania.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information:
Base Pay, depending upon qualifications and subject to negotiation:
$52.00 - $64.00 per point
