Job summary

Elara Caring in Worthington, OH is hiring a Registered Nurse to provide high-quality home health care. You will assess patients, update care plans, educate families, and coordinate with the care team to deliver compassionate, evidence-based nursing at the right time and place.

Ideal candidates have an active RN license, 1+ year of clinical experience, and the ability to lift up to 50 pounds. Travel between patient homes is expected; reliable transportation or a valid driver's license is required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to handle multiple priorities, documentation requirements, and deadlines
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred

• Complete initial evaluations and reassess nursing needs during follow-up visits based on patient, family, and facility needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Update the Plan of Care as patient needs change and coordinate revisions with the appropriate care team members.
• Educate and support patients, families, caregivers, and facility staff on the Plan of Care, hospice philosophy, and pain and symptom management.
• Collaborate with the Medical Director, attending physician, branch leadership, facility staff, and interdisciplinary care team to coordinate patient care.
