Job summary

Elara Caring is seeking a Registered Nurse – Home Health to join our team. You will assess patients, develop and implement care plans, and coordinate with physicians to deliver high-quality in-home care.

Responsibilities include ongoing patient assessments, education for patients and families, and maintaining HIPAA privacy while supporting safe, compassionate care. A valid RN license and strong clinical skills are essential.

Qualifications

• 1+ year of nursing experience in clinical care.
• Home Health experience preferred but not required.

Job Description:

To continue to be an industry pioneer delivering unparalleled care, we need a Registered Nurse Home Health with commitment and compassion.

Responsibilities:

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
