Simple Wellness in Baltimore, MD seeks a qualified Registered Nurse for a PRN home health role. You will perform Start of Care Assessments, coordinate care with physicians and therapists, and deliver hands-on nursing services in patients' homes.

The position requires 3+ years of hands-on home nursing, a current Maryland license, CPR certification, reliable transportation, and the ability to work independently in a fast-paced environment. Flexible work-from-home options available.

Qualifications

Bachelor's degree in nursing from an accredited program.
Current professional license in good standing.
CPR certification (American Heart Association).
Ability to lift up to 50 lbs and perform hands-on nursing in home care.
Must have reliable transportation and be able to work independently in a fast-paced environment.

Responsibilities

Perform Start of Care Assessments and Quarterly Reviews.
Oversee the needs of clients receiving services.
Document observations of patients’ condition and needs.
Provide home health care education to patients and families.
Coordinate care with physicians, physical and occupational therapists, aides, and others.
Provide hands-on nursing services (e.g., wound care, ventilator support, trach care).
Recommend strategies or devices to improve quality of life.
Take verbal orders and make referrals as indicated.
Oversee patient medication profiles and training sessions.
Complete service plans and investigate incidents or grievances.
