Job summary

Simple Wellness, LLC is seeking a dedicated Registered Nurse to provide hands-on home health care in the Baltimore area on a PRN basis. You will perform Start of Care Assessments, ensure care plans are followed, and educate patients and families on home health practices.

Responsibilities include coordinating with physicians and therapists, documenting observations, and responding to changes in patient condition.

Qualifications

• Bachelor's degree in nursing from an accredited program.
• Current professional license in good standing.
• 3+ years hands-on nursing in home care.
• CPR certification (AHA) required.
• Ability to lift up to 50 lbs.
• Reliable transportation.
• Background check must be clear.

Responsibilities

• Perform Start of Care Assessments and Quarterly Reviews.
• Oversee the needs of clients receiving services.
• Document observations of patient conditions.
• Educate patients and families on home health practices.
• Coordinate care with physicians, therapists and others.
• Provide hands-on skilled nursing services (e.g., wound care).
• Take verbal orders and make referrals as indicated.
• Oversee medication profiles and training sessions.
• Complete service plans and identify needs.
• Investigate incidents and communicate changes.
