Position Summary: Home Care RN (Mom/Baby)

The RN oversees and provides postpartum care and education to mothers and their newborns, as ordered by the physician and in accordance with the organization’s policies. Responsibilities include initial assessment, care plan development in collaboration with the Population Health Case Manager, providing functional support to Home Healthcare Aides and LPNs, and ensuring top client outcomes and patient satisfaction.
