As a Home Health Registered Nurse, You Will:

1. Provide admission, case management, and follow-up skilled nursing visits for home health patients.
2. Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
3. Confer with physicians to develop initial treatment plans based on orders and assessments. Provide hands-on care, management, and evaluation, and teach patients accordingly. Revise plans in consultation with physicians as needed.
4. Coordinate care with various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
5. Report patient care, condition, and progress to physicians and the Clinical Manager regularly.
6. Implement care plans with patients and families to achieve optimal outcomes.
7. Supervise and coordinate the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, including documentation and care instructions.
8. Discharge patients after consultation, completing all necessary documentation.
9. Maintain comprehensive medical documentation, including case conferences, patient contacts, medication changes, re-certifications, and updates.

Required Experience/Skills:

• Diploma, Associate or Bachelor Degree in Nursing
• Minimum of one year nursing experience preferred
• Strong med-surg, ICU, ER, or acute care experience
• Home Health experience is a plus
• Current and unrestricted RN licensure
• Current CPR certification
• Strong organizational and communication skills
• Valid driver’s license, auto insurance, and reliable transportation

Pay Range:

• $54.00 - $75.00 per visit/unit
• $84,600 - $116,300 per year base pay
