The RN in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies. The RN coordinates the total care plan with other healthcare professionals and maintains continuity of care by planning and exchanging information with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting.

Responsibilities include:

• Providing clinical services within the scope of practice, according to state laws and the plan of care.
• Conducting initial and comprehensive nursing evaluations, developing care plans, and documenting accurately and timely.
• Regularly evaluating and re-evaluating patient needs (at least every 60 days).
• Performing assessments and developing appropriate plans of care in collaboration with physicians.
• Ensuring patients meet home health eligibility and medical necessity guidelines.
• Initiating, developing, and revising care plans in collaboration with healthcare professionals.

**License Requirements:**

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation
