Job summary

Klarus Home Health is hiring dedicated Registered Nurse Case Managers to provide nursing services and case management to our patients receiving home health care in the San Antonio, TX area. The RN plans, organizes, and directs care focusing on community health and family needs within homes and communities.

Responsibilities include home visits, following care plans, wound care, medication administration, and documenting doctor orders with a state-of-the-art tablet.

Qualifications

• Current State or Compact RN license
• Minimum of one year clinical experience preferred
• Knowledge and skills in current nursing practice
• Valid state driver’s license and automobile liability insurance
• HCHB EMR knowledge is desired

Responsibilities

• Follow a designated care plan per patient needs
• Perform home visits to clients in designated territories
• Administer medications, wound care, treatments and procedures
• Monitor client conditions and report changes to Clinical/Client Services Manager
• Follow up with, execute, and document doctors’ orders
• Perform client assessments
• Coordinate case management and services
• Document observations and interventions using a tablet

Klarus Home Health is hiring dedicated and compassionate Registered Nurse Case Managers to provide nursing services and case management to our patients who receive home health services in the San Antonio, TX and surrounding areas. The registered nurse plans, organizes and directs home health care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Registered Nurse (RN) Responsibilities

• Follow a designated care plan in accordance with patient’s needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients’ conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors’ orders
• Perform client assessments
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications

• Current State or Compact Registered Nurse (RN) License in hand

• Demonstrate knowledge and skillset in current nursing practice
