Job summary

Pathways Healthcare is seeking an experienced Registered Nurse to join our home health and hospice team in the Manchester, NH area. The RN will assess patient needs, develop and revise plans of care, and collaborate with physicians to ensure high-quality, compassionate care.

Responsibilities include administering treatments, documenting care, and participating in interdisciplinary teams. 2+ years in nursing and a valid driver's license are required; training is provided for new nurses.

Qualifications

• Registered nurse with 2+ years of experience, preferably geriatrics.
• Strong bedside care and patient-family communication.
• Proficient with electronic medical records and CPR/BLS certification.

Responsibilities

• Assess and evaluate patient status and initiate care plans.
• Regularly re-evaluate patient and family needs and adjust plans.
• Administer medications and treatments as prescribed.
• Document clinical notes and report changes to physicians.
• Participate in interdisciplinary care meetings and on-call duties.

Position Details

• Main Office location: Manchester, NH
• Service Area: Hooksett and Surrounding areas.
• Job Type: Full Time
• Schedule: Monday to Friday

Responsibilities

Patient Care

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• New nurses, feel free to apply! We have a great training program.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

• Salary Range: $72K-90K Annual Salary
