Job summary

Pathways Healthcare in Massachusetts seeks an experienced Registered Nurse to join our home health and hospice team. You will assess patients, write plans of care, and coordinate with physicians to deliver high-quality, compassionate care in patients' homes, with a focus on rehabilitation and palliative support.

Full-time, Monday–Friday, Westwood office, serving Cambridge and surrounding areas. Competitive salary ($75k–$100k) plus comprehensive benefits and opportunities for leadership

Qualifications

• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient status by writing and initiating plan of care.
• Regularly re-evaluates patient and family needs and revises the plan of care as necessary.
• Develops a care plan with nursing diagnosis and palliative actions; involves patient and family.
• Administers medications and treatments as prescribed and documents care.
• Communicates changes to the physician and obtains orders as required.
• Provides a safe environment and attends interdisciplinary group meetings.
• Participates in on-call duties and ensures equipment and services are available.
• Supervises ancillary personnel and supports professional growth.

Position Details:

• Main Office location: Westwood, MA
• Service Area: Cambridge and Surrounding Areas
• Job Type: Full Time
• Schedule: Monday - Friday
• Salary Range: $75,000.00 - $100,000.00

Responsibilities:

Patient Care:

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
