Job summary

Pathways Healthcare in Exeter, NH is seeking a full-time Registered Nurse to deliver high-quality in-home care in Raymond and surrounding areas. You will assess patients, draft and update plans of care, and coordinate with physicians to ensure safe, compassionate treatment in the home setting.

The role emphasizes patient and family involvement, on-call duties, and ongoing professional development. Monday–Friday, day shifts, with a comprehensive benefits package and supportive team.

Qualifications

• Nursing degree from an accredited institution with RN license.
• 2+ years of nursing experience, preferably geriatrics.
• Proficient with electronic medical records (EMR).
• Excellent bedside manner and communication skills.

Responsibilities

• Assess and evaluate patient status and develop a plan of care.
• Regularly re-evaluate patient and family needs.
• Revise the plan of care as necessary.
• Develop goals-based care plans with patient/family involvement.
• Administer medications and treatments as prescribed.
• Maintain accurate clinical notes and report changes to physicians.
• Provide a safe home environment and participate in meetings.

Position Details:

• Main Office location: Exeter, NH
• Service Area: Raymond and surrounding areas.

Responsibilities:

Patient Care:

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
