BAYADA Home Health Care in Prescott, AZ is seeking a Registered Nurse (RN) to join our home health care team, delivering skilled nursing and rehabilitative services in clients' homes.

You will perform assessments, educate patients and families, administer medications, complete documentation, and participate in weekend on-call rotations.

Qualifications

• Arizona RN license is required.
• Graduation from an accredited nursing program as indicated by transcript or diploma.
• 1 year nursing experience required.

Prior home health and OASIS experience required.

Taking part in weekend on-call rotation required.

How You'll Make an Impact:

• Deliver 1:1 skilled nursing, intervention, and supportive care in accordance with individual care plan, including administration of medication, wound care and treatments

• Perform client assessments, provide education to the patient and/or their family, and monitor patients' condition.

• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered BAYADA Nurse:

• A current nursing license in good standing in the state of Arizona

• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma

• Must have 1 year nursing experience
