Mental Health Association of Rochester seeks a weekend Home Care Registered Nurse to provide compassionate nursing in patients’ homes. You will assess health needs, develop care plans, administer medications, and document care while coordinating with physicians and the care team to ensure continuity of care.

Educating patients and caregivers on safety and treatment plans is key, with emphasis on honoring patient preferences and goals in the home setting.

• Active RN licensure required.
• Strong clinical judgment and ability to work independently in the home setting.
• Clear, empathetic communication with patients, families, and care teams.

• CPR / BLS certification preferred.
