MedCadre is seeking an experienced Home Health RN in Oregon to provide patient-centered care in a skilled home health setting. The role features day shifts (8 hours) with a 1:1 patient ratio and expected travel up to ~60 miles per day.

OASIS and HCHB documentation are part of daily responsibilities, and current Oregon RN licensure is required before start date. The position requires at least 1 year of home health experience, BLS, reliable transportation, and willingness to support weekends.

• Shift: Day (5x8 Hours) 8:00 16:30
• Description: Assignment Details
• Schedule: 8-hour day shifts
• Schedule Restrictions: 4x10s and 3x12s are not available
• Assignment Setting: Skilled Home Health
• Patient Ratio: 1:1
• Productivity: Approximately 30 points per week; up to 6 patients per day
• Experience Required: At least 1 year of skilled Home Health experience
• First-Time Travelers: Accepted
• Weekend Requirement: Weekend support may be required
• Call Requirement: None Required
• Certification: BLS
• Local Clinicians: Accepted within a 5075 mile radius at the same rate
• License: Active Oregon RN license required before the start date; temporary licenses are not accepted
• Special Requirements: Clinicians must have reliable personal transportation. The facility makes every effort to group patient visits and minimize driving. The service area may occasionally extend up to approximately 60 miles, or 6090 minutes of total driving per day.
• OASIS experience is required.
• Home Care Home Base (HCHB) experience is required.

• Facility Information: Facility Type: Home Health Attire: Business casual; no jeans. Scrubs are permitted but not required, and there is no specific scrub color. Parking: Free Mileage: Reimbursed at the current IRS standard rate
